To register your student please follow the following directions.

NOTE: Please be sure to read all steps and directions below. All steps must be completed to make sure your child is properly registered.

Once you have completed the forms, please watch your email inbox for instructions on how to complete the registration process. All communication will take place via email.

Kindergarten Survey (English)

MANDATORY for all students entering or returning from out-of-district schools: Records Release Form

STEP 2 - Send a scan or photo of each of the required documents via email to or via fax to 413-447-7805

Gather the following REQUIRED DOCUMENTS:

• Valid PARENT or LEGAL GUARDIAN Photo ID: State-issued ID, Driver's license or Passport
• Birth Certificate
Last physical exam record
Proof of Residence—such as a current utility bill or rental agreement
• Middle and high school students—the student’s transcript and most recent grades
• Students with an IEP—a copy of the student’s Individual Education Plan
